Ji Han MD
Dr. Ji Han is a board certified anesthesiologist who specializes in pain management. He received his degree at University of Alabama at Birmingham in 2002. He has more than 20 years of professional experience in Pain Management treatment. He was the Director of Pain Management at New York Presbyterian Hospital-Queens and also the Director of Pain Management at Queens Medical Associates Center. Dr. Han’s passion is to provide the most personal and genuine care for his patients as he understands the complexity of pain.Most importantly, Dr. Han performs evidence based approach in pain management and tailors his treatment plan to his patients individually, rather than a “one size fits all” approach. Dr Han received his residency training at New York Presbyterian Hospital- Weill Cornell Medical Center and fellowship training at New York Presbyterian Hospital-Weill Cornell Medical Center, Hospital for Special Surgery, and Memorial Sloan Kettering Institute.

What Information do I need when scheduling an appointment?Patients full name, date of birth, current address, phone number, current insurance information, purpose of appointment, and information on where you have taken any radiology imaging.

What do I need for my appointment?Please bring your driver’s license, medical insurance card, list of all current medications, all radiology reports such as MRI, X-RAY and EMG results.

I tried to call your office but I can’t reach your medical staff member. What do I do?If you are unable to reach a live person, please leave a message with your name, phone number, date of birth and a contact phone number and we will return your phone call ASAP.

How do I refill my prescription?The fastest way to get medication refill is contact us at 718-762-7000, press 1 for English and press 3 for medication department. If you are not able to reach someone, please leave a message with your name, date of birth, and phone number and one of our staff will get back as soon as possible. Please understand that we do not handle prescription related matters on weekends.

What do I do when I need a form completed?Please allow 2-3 weeks for our office to complete forms. Forms can be shared with us via our fax number: 718-762-7002, or dropped off at our office. There may be a fee associated with completing the forms.

How can I obtain my medical records?To request your medical records, you may ask our front desk staff directly or email our medical record department at mr@NYSChealth.com. If you are trying to obtain your medical records to send to another physician office, you should ask the doctors office to send us a HIPAA request to our fax number (718-762-7002), and we will send over your records as soon as possible.

What services do you perform?We are an interventional pain management office. Our services include conservative treatment including: medications such as non steroid anti-inflammation, nerve medication, muscle relaxant, etc. Treatments including epidural steroid injections, radio-frequency ablations, medial branch blocks, percutaneous discectomy, spinal cord stimulator trials, doral root ganglion trials, and trigger points are also performed.

Will I be asleep for the procedure?You will be slightly sedated with anesthesia for the duration of the procedure, which is relatively about 10 to 15 minutes.

Will someone need to pick me up?Yes, you must bring an escort with you on the day of the procedure to take you home.

What are the instructions before the procedure?For most of our procedures including: spinal cord stimulator trials, epidural injections, and nerve blocks you are given instruction to not eat or drink anything for 6 hours. If you are on blood thinners, you will need your primary care doctor or the specialist who is prescribing the medications to sign a consent to stop blood thinners 3-7 days before the procedure depending which blood thinner you are on.

If I don’t feel well after my procedure and office is closed, can I reach the doctor?Yes, of course! Our providers will be available after hours. Please call our office phone number 718-762-7000 to reach our physician after hours, and leave a message with your name, date of birth and phone number and the provider will call you back as soon as possible. However, in the event of an emergency, please call 911. Please note that we do not handle prescription related matters on weekends.
